Custom Skates

Before I start off, let me say that I've used the "search" feature, but couldn't find any answers to my questions. They're probably out there, I just didn't come across any. Anyhow, to get to my questions.

1. Aside from material choices, is a mold taken of your foot in order to ensure exact fit?

2. Do you get to to look at some material samples to choose from? In other words, how do I know what materials are available to choose from, and the pros and cons of each.

3. If needed, can I just order the boots w/out the holder/blade? I know this answer may vary depending on manufacturer

I just ordered some custom pro tacks and the options I was given were size (left/right, length and width), ankle support, tendon guard, tongue, lining, toe cap, special facing (not sure what this means) and blade size. No mold was taken of my foot and I don't think that is an option. I didn't look at any material samples, but I didn't ask to because I already knew what I wanted.
