Fair Play shift rotation downloads. Help!

I am coaching house league where there is a fair play rule in effect. I coached one year with a coach who had a form on which you just plugged in the names of the kids and the rotation was all figured out. There was a separate form for different numbers of players so if two kids weren't there, you used the sheet with the appropriate number and everything got sorted out on who sat when etc. This coach isn't around anymore and for the life of me I can't find anything similar on the internet. Anyone know where I can get something like this that will work on a Mac? Thanks in advance.
