Reffing Penalty Question

I misplaced my rulebook, and this has been bothering me all day. Player gets three sets of penalties called on him in one game: a 2 minute minor for cross checking, a 4 minute double minor for cross checking, and a 2 minute+10 minute misconduct for check from behind. Does said player technically have 5 penalties, thus giving him a game misconduct? Does each call on the double minor count as a penalty, or is it just that considered just one penalty? Same with the 2 and the 10, are each counted separately or grouped as one penalty? Said player's 10 minute misconduct happened within the last 10 minutes of the third period, so he was sent to the locker room early, but not given a game misconduct by the ref. It was also not marked on the scoresheet if a game misconduct was handed out, as well.

According to USA Hockey, there is no option for a double minor for crosschecking, it can't be called. Roughing is the only offense that can warrant a double minor. Everything else is 2 or 5 or 5+ Game.

That said, each minor in the double is considered a penalty. So 2+4+2+10 is actually 2+(2+2)+2+10, or 5 penalties.

I'll have to look when I get home and see how the game is handled, I don't recall.
