"third eyelet test" question

I've done a search, but haven't been able to find the exact answer--the test is done with the tongue pulled out, correct? If so, how much of a "bulge" is still OK? 1/4"? 1/2"? Anything above absolute flat is not OK?

I use that same standard when picking up strange women in bars. You REALLY don't them to have a "bulge"......
