In the past, I've used Big Stick Hockey. They're located in Toronto but they have no problems shipping. I don't know if you're looking for someone to design the jersey as well (I designed the jerseys for my team so the process was relatively painless - just handed them the Illustrator files along with the fonts I wanted) but they're really nice guys and they'll work with you to create whatever jersey you want.

Stingers Sports on St-Johns boulevard in Pte-Claire, is where a lot of teams have their jerseys done.

They make pro jerseys, replicas and custom.

Their shop as some shoes, skates and sticks, but their main business is making jersey's and uniform.

I've used these guys before, Silvestri Sports. They make everything in-house, so you can really go completely custom if you want to. They're also fast- they did 25 custom tackle twill jerseys for my team in 2 weeks.
