Policy and Procedure form for warranty...

I've decided to ship my stick with the chipped blade the Hockey Company. They already gave me a return authorization # and asked for this...

IMPORTANT INFORMATION :

The following procedures and / or documents are mandatory in order for

your request to be processed:

. Your proof of purchase.

. Your One-piece stick/shafts warranty Policy & Procedure form must be

filled out received on

the day of purchase.

. Your return authorization number must appear on your parcel.

. Please do not cut your stick in pieces to facilitate shipping as

this will void your warranty.

Where the hell can I find this? I did not see it anywhere in the packaging when I got the stick.

No, they are not the same form. If you have a LHS go in and, very kindly, ask them if they would be so kind and give you a form.

I don't know about LHS, but I have a Perranis next to me. So should I just ask them for one of those forms?

I was actually just about to ask this question. Why don't online hockey shops include this when you buy a stick?

Also if anyone has a printable form of it and they can send it to me it would be much appreciated.

No, they are not the same form. If you have a LHS go in and, very kindly, ask them if they would be so kind and give you a form.

I don't know about LHS, but I have a Perranis next to me. So should I just ask them for one of those forms?

haha, I thought it was the name of some major hockey chain that was located in everywhere but Pittsburgh.
